Veterinary Referral/Consent

 

Due to legislation and professional requirements a veterinary referral is required before assessment/treatment can be provided.

 

Veterinary professionals are able to access the referral form here and return to me by either email or post.

 

You are able to self-refer your animal however veterinary consent will be sought prior to the appointment. Please either ask your vet to complete the referral form or contact me and I can arrange this for you.
